New Procedures for Dispensing of Medicines

Dear
Parents,
In an effort to follow new "best
practices" concerning the distribution of prescription and non-prescription
(over the counter) medications to students, Calvary Road Christian School will
be implementing the following policy.
As of January 1, 2008, we will no
longer be able to administer any medication (prescription or over the counter)
without a specific consent form from the Department of Social Services. This
form must be signed by a parent or legal guardian and is attached for your
convenience. It will enable us to give a prescription and non-prescription
medication for up to ten days. It will
also be available in the school office and online at our website.
If the medication is to be given
longer than 10 days, the form must be signed by a physician and is then good
for up to six months. After six months a new form must be signed. Please
remember, this is for non-prescription medication as well. Due to this change,
we will no longer be able to hand out over the counter medicines such as
Tylenol, Benadryl, or cough drops without a signed form in advance.
Any medication (prescription or
non-prescription) must be provided directly by the parents with the form and
must be in the original package with the child’s first and last name clearly
labeled. The "name of medication" box on
the consent form must match exactly with the medication brought in. All
prescription medications must also contain the print out received from the
pharmacist that lists side effects and what it is for. A form will go home
stating the date and time a child has received any medication and a specific
record will be kept in the office.
Parents will be notified to pickup
any medications after the consent form runs out. After 14 days all medicines
will be disposed of if not picked up.
Any child that has special health
care needs (attention deficits, seizures, asthma, diabetes, serious allergies,
etc.) must fill out an "Individual Health Care Plan" which is available in the
office.
